HUMS 215 Field Work Practicum Time Log
Maintain this time log throughout your practicum and submit to your instructor at the end of the semester. The site supervisor may initial the hours as you complete them, or sign off on the completed hours at the bottom of the pages, in order to verify hours. You may use another type of document to account for time as long as it can be provided to the Instructor.
